Forward Grouping (Cell Grouping)
Definition
The direct test of red blood cells with known anti-A and anti-B reagent antibodies to determine which ABO antigens are present on the cell surface. Agglutination of cells by anti-A indicates the A antigen is present.

- Antigens on the red cell surface
- Reagents used
- Known anti-A and anti-B antisera
- Paired with
- Reverse (serum) grouping for verification
Common questions
Why is forward grouping normally paired with reverse grouping rather than used alone?+
Forward grouping tests the cells against known antibodies, while reverse grouping tests the serum against known cells. Running both and confirming they agree catches typing errors, weak antigen expression, and rare subgroup variants that a single test could miss.
How does forward grouping apply to forensic bloodstain analysis rather than clinical transfusion medicine?+
In forensic casework forward grouping helps establish ABO type from a bloodstain when reverse grouping is not feasible because degraded or dried stains rarely retain functional serum antibodies, so the cell-surface antigen test carries more of the identification burden.
What can cause a false or ambiguous forward grouping result on a degraded sample?+
Bacterial contamination, prolonged storage, and antigen degradation can weaken or alter agglutination reactions on old or environmentally exposed stains, which is why forensic serologists interpret weak or inconsistent reactions cautiously and may repeat testing with fresh extraction.
