Why test for this metabolite instead of the parent drug itself?+
Flunitrazepam is rapidly and extensively metabolised, so unchanged parent drug drops below detectable levels quickly. The metabolite persists in urine much longer, making it the practical target for a drug-facilitated sexual assault workup.
Will a routine benzodiazepine immunoassay pick this up reliably?+
Not reliably. Standard immunoassay cut-offs are calibrated against diazepam metabolites, and 7-aminoflunitrazepam does not cross-react well with them, which is why confirmatory LC-MS/MS testing is needed.
